On-Going Client Monitoring

Proctor's Ongoing Client Monitoring allows families to rest assured that a qualified health professional is constantly monitoring the health of their loved one. The Care Manager will regularly re-assess the client to ensure the care plan is meeting all of their health needs. Families will receive regular progress reports outlining recommendations and changes designed to ensure the continued health and safety of their loved one.
The Care Manager will also liaise with other health professionals involved in the elderly clients care to ensure a colloborative approach to identified health issues.
Typically On-Going Client Monitoring might include the following;
1. Evaluation and adjustment of the care plan to ensure optimum health and well-being.
2. Liaising with other health professionals to ensure identified health and safety issues are been effectively managed. This includes communicating with the family Doctor.
3. Providing 24 hour a day crisis management.
4. Regular family updates providing an overview of the clients functioning and recommendations.
5. Ensuring medications are been taken, that appropriate medical appointments are made, that care supports are adequate for the identified health needs.
