Proctor Care Managers have extensive experience and knowledge navigating the health care system. Care Managers are able to assist families communicate with the various health professionals involved in a clients care. Your Care Manager will establish clear lines of communication with the care team, which enables families to feel confident and well informed to make health care decisions.
Typically Client advocacy might involve;
1. Liaising with the health care team in order to provide family with the most up to date health information regarding their elderly loved one.
2. Ensuring the health care team is aware of the client and family goals.
3. Advocating on behalf of the client and family to ensure that all available supports and services are accessed.
4. Facilitating a smooth transition from hospital to home by implementing appropriate transitional services to ensure a clients ongoing safety and independence.
5. Ensuring all required equipment is in place for discharge home.
6. On-going support from the Care Manager as required.
