Proctor Charges an hourly rate of $85. All service begins with a 2 hour initial assessment.
Several packages are available which families may find more cost effective.
Basic Care Plan
1. Proctor completes a comprehensive assessment of the clients health status and home environment.
2. Proctor creates a care plan tailored to the individual clients needs. This includes recommendations for home services, home safety equipment, medication review, and links with community services.
3. Once a month review of health status with accompanying report and recommendations.
Cost$80 per month.The basic care plan is for families who live in the local area and may be available to assist with implementing some of Proctor's clearly outlined recommendations. Comprehensive Care Plan
1. Proctor completes a comprehensive assessment of the client's health status and home environment.2. Proctor
creates a care plan tailored to the individual clients needs. This

medication review, and links with community services. Proctor's team of health professionals implement the plan under the direction of the family.3. The Care Manager visits the client weekly making changes to the care plan as required. A monthly report is provided to the family.
4. The Care Manager attends medical appointments and liaise closely with the family Doctor regarding the client's medical management.5. The Care Manager is available for 24hr emergency input.Cost$150 per month.For those families who have limited time due to work and other commitments or simply live in a different geographical location the Comprehensive Care Paln is for you.
