Transitional Assistance

"Proctor understands that as people age their care requirements can also change. The transition to alternative housing can often be a daunting experience for families and their elderly loved ones"



Proctor assists families with the process of finding alternative housing options for their elderly loved one.  The Care Manager will consider the clients personal preferences, care requirements, and financial situation when identifying housing alternatives.  The Care Manager will then support the transition ensuring identified care needs are met, that counselling services are provided, and moving logistics arranged.

Typically Transitional Assistance might include;

1.  Comprehensive assessment of a clients physical, cognitive, and social functioning.

2. Detailed analysis of available housing options for family considering each client's needs.

3. Counselling services for family and client to assist during the transition process.

4. Assistance with transitional logistics, such as downsizing and moving.

5. Establishing a care plan in the new housing environment to ensure adequate supports and services are in place to facilitate independence and safety.

6. Recommendations for equipment and aids to assist the client in their new home.

7. On-going support as required.
